MALE PT UNDERGOING VERTEBRAL INTERBODY FUSION. ATTEMPTED SPINEOLOGY BONE BAG SYSTEM. INSTRUMENTATION BROKE, UNABLE TO RETRIEVE STEEL SHARDS FROM FRACTURED ACCESS PORTAL. INSTRUMENTATION FAILURE LEADING TO RETAINED STEEL SHARDS/PIECES IN SPINE. (WITH IN L5-S1 DISK SPACE).
