Description of Event or Problem
ACCORDING TO THE REPORTER, DURING LAPAROSCOPIC HIATAL HERNIA REPAIR, THE DEVICE DID NOT GRASP PROPERLY AND THE GRASPER FELL INTO THE CAVITY OF THE PATIENT. SURGEON REMOVED IT WITH ADDITIONAL GRASPERS. THERE WAS NO PATIENT INJURY.
