WEBCOL (COVIDIEN) REF (B)(4): 70% ALCOHOL PREP PAD OPENED IN IV ROOM. FOREIGN SUBSTANCE FOUND INSIDE PREP PAD PACKAGING PRIOR TO USE. PAD WAS NOT USED. PACKAGE, FOREIGN MATERIAL AND PAD PLACED IN PLASTIC BAGGIE.
