WHILE USING SAGE KIT FOR MOUTH CARE ON VENTILATOR, THE SWAB BROKE OFF INTO PATIENTS MOUTH. I REACHED INTO HIS MOUTH TO REMOVE THE SWAB. NO HARM TO PATIENT.
