SMALL SILICONE WASHER FELL OUT OF THE TAUT INTRODUCER PERITONEAL CATHETER INTO THE PT'S ABDOMEN. SURGEON WAS ABLE TO RETRIEVE THE WASHER. ONLY WASHER / SEAL AND PACKAGING RETAINED BY STAFF.
