Description of Event or Problem
THE CUFF COVERING THE TUBES PERFORATED. DISTAL-END REMAINED IN THE PATTENT'S ESOPHEGUS WHEN THE TUBE WAS REMOVED. STAFF WAS ABLE TO RECOVER THE CUFF AND REMOVE IT. THERE WAS NO PT HARM REPORTED.
