DURING APPLICATION OF A HALO CROWN AND JACKET ON A PT, IT WAS NOTICED THAT ONE OF THE HIFIX HALO PINS HAD MANAGED TO PENETRATE THROUGH THE PATIENT'S SKULL WITHOUT THE TORQUE CAP BREAKING AS INTENDED.
