Description of Event or Problem
PICC LINE CLOTTED, RETAVASE WAS USED TO OPEN THE LINE. THE IV LINE WAS SUCCESSFULLY OPEN. FACILITY'S ADVANCED PRACTICE NURSE WILL BE EVALUATING THESE OCCURRENCES.
