Description of Event or Problem
CATHETER PASSER, DISPOSABLE, 60CM WAS USED IN A VP SHUNT CASE. THE SAID ITEM USED WAS DEFECTIVE DUE TO EXCESS SPACING WHEN THE CATHETER WAS PLACED INSIDE.
