Description of Event or Problem

WHILE DRAWING BLOOD, THE BLOOD LEAKED FROM THE PROXIMAL END OF THE VENIPUNCTURE TUBING AND LEAKED ONTO THE PATIENT'S CLOTHING. THE TUBING WAS DEFECTIVE AND HAD A LEAK.
