Description of Event or Problem

ADHESIVE PORTION OF COVADERM PLUS VAD DRESSING IS PULLING / TEARING SKIN. SITES CLEANED WITH CHLOROHEXADINE AFTER REMOVAL OF DRESSING. PTS COMPLAIN OF STINGING SENSATIONS AFTER CLEANING. THIS OCCURRED DURING DRESSING FOR BONE MARROW TRANSPLANT / CHEMOTHERAPY PTS.
