Description of Event or Problem
THE SURGEON REPORTED THAT WHILE USING A PLASTIC MST PHACO WRENCH PLASTIC WAS DEPOSITED ON THE PHACO NEEDLE WHICH THEN INTRODUCED THE PLASTIC INTO THE PT'S EYE. THE PLASTIC PIECES WERE REMOVED EASILY.
