Description of Event or Problem
PT WAS USING TRAPEZE BAN TO PULL HERSELF UP IN BED AFTER SURGERY. THE CHAIN SEPARATED AND BROKE FREE FROM SYSTEM CAUSING BAN TO COME LOOSE. NURSE CAUGHT BAR, NO PT HARM.
