Description of Event or Problem
THE PHYSICIAN WAS REMOVING THE ABLATION CATHETER FROM THE VERSA CROSS SHEATH WHEN IT WAS NOTICED A CLEAR THREAD-LIKE PLASTIC HANGING OUT OF THE HUB. IT WAS PULLED AND A LONG PIECE OF IT CAME OUT.
