Description of Event or Problem
SURGEON WAS COMPLETING A PROCEDURE FOR INTERNAL BLEED AND WAS UTILIZING THE SPEED BAND FROM BOSTON SCIENTIFIC. AS THE SURGEON WAS DEPLOYING THE DEVICE INTRAOP THE BANDS GOT ALL TWIST AND WOULD NOT DEPLOY.
