Description of Event or Problem
DISPOSABLE SIGMOIDOSCOPE WAS BEING USED, AND THE "WINDOW" POPPED OUT CAUSING INSUFFLATION TO BE LOST. SURGEON NOTED THIS HAS HAPPENED MULTIPLE TIMES. NO HARM TO THE PATIENT. A NON-DISPOSABLE SIGMOIDOSCOPE WAS USED TO COMPLETE THE EXAM.
