Description of Event or Problem
MICRO TECH COLD SNARE OPENED IN PATIENT- NOTICED THE TIP OF SNARE WAS BENT AND FOLDED OVER ONTO ITSELF. DEFECTIVE DEVICE, SNARE WAS REMOVED FROM PATIENT, WASTED AND REPLACED WITH A PROPER WORKING SUPPLY.
