Description of Event or Problem
A TISSUE CASSETTE WAS NOTED TO HAVE NO TISSUE INSIDE. A PIECE OF THE CASSETTE WAS NOTED TO BE MISSING. ALL BIOPSY CASSETTES WILL BE TAKEN OUT OF SERVICE AND TISSUES CASSETTES WITH SPONGES WILL BE USED.
