EMPLOYEE WAS CLEANING THE DAILYSIS CHAMPION CHAIR WITH A 1% BLEACH WIPE AND SUDDENLY HER FINGER WAS PINCHED IN THE HINGE ASSEMBLY. RIGHT INDEX FINGER.
