Description of Event or Problem
A PIECE OF AN INSTRUMENT BROKE AND FELL INTO A PATIENT¿S BLADDER. WE WERE UNABLE TO RETRIEVE THIS PIECE OF INSTRUMENT. THE INSTRUMENT IS A COLD CUP BIOPSY FCPS.
