DISPOSABLE INSTRUMENT WAS SET-UP BY SCRUB AND REP, WHEN SURGEON HAD "JET ALIGNMENT" SET FOR AQUABLATION PART, INSTRUMENT FAILED TO WORK. NEW DISPOSABLE INSTRUMENT OPENED AND SET-UP WHICH WORKED WITH NO PROBLEM.
