Description of Event or Problem
CEILING MOUNTED X-RAY LEAD SHIELD FELL OFF THE ATTACHMENT ARM WHILE PT WAS ON THE TABLE, DURING AN ANGIOGRAM CASE. SHIELD FELL TO THE FLOOR AND BROKE. NOBODY WAS HURT.
