THE RAPTOR DEVICE WAS BEING USED DURING A PROCEDURE AND ONCE DEVICE WAS DOWN THE SCOPE, THE DEVICE FAILED TO "OPEN." DEVICE WAS THEN TAKEN OUT OF THE SCOPE.
