Description of Event or Problem

WHILE THE RADIOLOGY TECH WAS WIPING THE PATIENT'S ARM WITH IV START KIT SPONGE, THE PATIENT BEGAN TO BLEED FROM SCRATCHES CAUSED BY A SMALL PIECE OF PLASTIC OR GLASS IN THE SPONGE.
