Description of Event or Problem

WHILE ULTRASOUND PROBE WAS BEING USED IN STERILE FIELD, THERE WAS A HOLE NOTICED IN STERILE ULTRASOUND DRAPE. PROBE COVER WAS DISCARDED, VANCOMYCIN POWDER ADDED TO ONE LITER OF IRRIGATION.
