Description of Event or Problem
PER MEDWATCH REPORT # (B)(4) RECIEVED, THE GRASPER BROKE DURING FIRST ATTEMPT TO GRASP A URETERAL STENT. THE PIECE OF JAW THAT BROKE OFF WAS RETRIEVED AND REMOVED FROM THE PATIENT.
