ENDO CATCH 11 SPECIMEN RETRIEVAL POUCH WAS BEING USED BY PHYSICIAN TO EXTRACT SPECIMEN FROM ABDOMEN AND THE BOTTOM OF POUCH BROKE AT THE SEAM. FDA SAFETY REPORT ID# (B)(4).
